Order Form

All orders due to your team admininstrator by Aug 7, if he/she needs to adjust the turn-in date.  So please

verify everything with your team administrator.  To team admins:  all orders/checks due to Jeff D by Aug 12 - see info at bottom for more info.

 
The decals will be done in Sept (date TBA).  This will be the only order before Jan '10.

For Administrators

Please let your team parents know that if they have not yet ordered, they can fill out a form (download above) with payment to you by Aug 7 (checks payable to WASA Eagles).
 
Then I will need to receive all orders and payments from you by August 12 (they can be dropped at my office at Northside--it would work well to handle all of this at team camp).  Please wait to turn in your whole team order together
