Here is the checklist of items that team administrators need from every Eagle parent:

  • Team info sheet (downloadable form below)
  • Player form (downloadable form below)
  • Uniform order - due xx/xx/10 (TBA) - see below for more
  • Medical release (downloadable form below)
  • Birth certificate copy
  • 1x1" photo
  • Check for fall fees, payable to WASA Eagles FC ... notes:  1)  uniform money will be due in early August, and 2)  please notify your team admin if you have requested scholarship funds from Jeff Dybdahl (contact Jeff at dybby@juno.com for all scholarship requests)
